Job Title:

We are currently seeking positive, upbeat, outgoing candidates for the position of Targeted Case Manager (TCM). As a Targeted Case Manager, you will make a difference each day helping individuals with a Severe and Persistent Mental Illness or Severe Mental Illness to live full lives. You will help individuals access needed medical, social, educational, and other resources that may help them be successful in living independently. These services are provided in both agency and community settings. In addition to monitoring treatment plans and progress notes to assure the plan is meeting the needs of the client. The ideal candidate has a passion for behavioral health, is creative, possesses strong organizational skills, time management skills and has an awesome interpersonal style.  
GENERAL RESPONSIBILITIES:  
• Provide Targeted Case Management (TCM, OneCare) to persons assigned to caseload.
• Will complete assessment to determine the need for any medical, educational, social, or other services.
• Help individuals gain/maintain access to needed medical, social, educational, and other services.
• Gather information from other sources as needed.
• Work hours are flexible to meet the needs of the person served.
• Contact with new referral is made within 48 hours of assignment.

ESSENTIAL JOB FUNCTIONS: 
• Will make all contacts as indicated in the Tx plan.
• Progress notes are completed within 24 hours of service.
• Makes appropriate referrals to CM/PS/PRI/SE and completes required paperwork.
• Complete Social Determinants of Health, Tx Plan, and any other assessment within the required timeframe.
• Will monitor and follow up to ensure member needs are being addressed.

MINIMUM QUALIFICATIONS:  
• Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.
• Excellent interpersonal and teamwork skills 
• Excellent organizational and time management skills 
• Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families. 
• Have completed or will complete any required training within six months of employment.
• Must have a current and valid Kansas driver’s license.
